The Enrollment Assistance Program (EAP) has come to an end. At this time, we are no longer providing assistance with our certified Enrollment Assisters.
If you need health coverage, submit a new or updated Marketplace application right away to see if you (or other members of your household) can buy a Marketplace plan and get help with costs. It only takes a few steps to see if you can get covered. Visit HealthCare.gov to get started.

You can find local help to assist you and other household members enroll in coverage. Local Marketplace Assisters provide free and impartial enrollment assistance. You can get more information about help in your local area at LocalHelp.HealthCare.gov.

Work with an EAP Assister to schedule an appointment
Prepare for the appointment by gathering required information
Be on time for your scheduled appointment (or reschedule in advance if needed)
Let your EAP Assister know if you need an interpreter or other accommodation for assistance
Help you better understand your healthcare options and answer your questions regarding healthcare
Guide you through the application and enrollment process
Direct you to resources you may need to overcome barriers preventing you form gaining healthcare coverage
Connect you with a Navigator in your area for in-person or virtual assistance
EAP conducts direct assister-to-consumer outreach alongside Navigator grantees in all 28 Federally-facilitated Marketplace (FFM) states (shaded in blue), to help consumers maintain continuity of coverage.

Cognosante employs certified Enrollment Assisters. The Centers for Medicare & Medicaid Services (CMS) provides a certificate for each trained, certified Enrollment Assister.
Your Assister will provide their CMS certificate upon request. It looks like this certificate and contains their unique Enrollment Assister assigned ID number.
